Privacy Policy
Your privacy matters to Northgate 55.
Northgate 55 is committed to respecting and protecting your privacy. This Privacy Policy explains how personal information may be collected, used, stored, and shared when you visit our website, contact Northgate 55, book a service, or receive services from us.
We aim to collect only the information reasonably necessary to provide services, respond to inquiries, manage our business, and communicate with you.
Information We May Collect
Northgate 55 may collect personal information that you choose to provide when you contact us, book a service, participate in a consultation, or otherwise receive support from Northgate 55.
Depending on how you interact with us, this may include:
Contact information — such as your name, email address and telephone number.
Booking and service information — such as the service you request, appointment details, communication preferences and information reasonably necessary to arrange or provide the requested service.
Information you choose to share — during consultations, navigation, advocacy or appointment accompaniment, you may choose to share information about your circumstances, needs, health, family situation, services you receive, or other personal matters relevant to the support you are requesting.
Information about another person — family members or caregivers may sometimes provide information about someone they are supporting. We ask that you share only information that is reasonably necessary and that you have appropriate authority or consent to share whenever required.
Northgate 55 is a consent-based service. A family member or caregiver may contact us to ask questions or explore possible supports; however, Northgate 55 will not provide services directly to another capable adult without that person's consent.
Website and technical information — our website and service providers may automatically collect certain technical information, such as device or browser information, website activity, cookies or similar technologies used to operate, secure and understand use of the website.
Northgate 55 aims to collect only the information reasonably necessary for the purpose for which it is being provided.

Northgate 55 may use personal information for purposes such as:
-
responding to questions and inquiries;
-
scheduling, confirming, rescheduling, and managing appointments;
-
providing the services and support you have requested;
-
communicating with you about your services or next steps;
-
maintaining appropriate business and service records;

-
improving our services and website; and
-
meeting applicable legal, regulatory, professional, or administrative requirements.
Northgate 55 will not use personal information for purposes unrelated to why it was collected unless we have your consent or are otherwise permitted or required by law.
Marketing Communications
Northgate 55 may occasionally offer newsletters, educational updates, resources, or information about our services.
Providing your email address to ask a question, book a service, or receive support does not automatically subscribe you to marketing communications.
Where consent is required, Northgate 55 will obtain appropriate consent before sending marketing communications. You may unsubscribe from marketing emails at any time using the unsubscribe option provided or by contacting Northgate 55.
Storage, Protection & Sharing
How We Store & Protect Your Information
Northgate 55 takes reasonable administrative, technical, and organizational measures to protect personal information against unauthorized access, use, disclosure, loss, or misuse.
Personal information may be stored or managed using secure third-party service providers that support Northgate 55's operations, such as website hosting, booking, client management, email, video conferencing, and payment processing services.
Northgate 55 limits access to personal information to those who require it for legitimate business or service purposes.
No method of electronic communication or storage can be guaranteed to be completely secure. We therefore encourage clients not to send highly sensitive personal, health, financial, or identification information through general website forms, regular email, or social media.
Sharing Your Information
Northgate 55 does not sell or rent your personal information.
Personal information may be shared with service providers that help Northgate 55 operate its website, manage appointments, maintain appropriate client records, process payments, communicate with clients, or provide other necessary business functions. These providers receive information only as necessary for the services they provide and are subject to their own privacy and security practices.
Information may also be disclosed when you have provided consent, when reasonably necessary to provide a service you have requested, or when disclosure is permitted or required by law.
When Northgate 55 is supporting you in communicating with another organization, professional, or service provider, we will seek your consent before sharing your personal information unless disclosure is otherwise permitted or required by law.
Consent & Withdrawing Consent
Northgate 55 seeks meaningful consent for the collection, use, and disclosure of personal information, except where otherwise permitted or required by law.
Consent may be provided verbally, electronically, in writing, or through another appropriate method depending on the circumstances and the sensitivity of the information involved.
You may withdraw your consent at any time, subject to legal, contractual, professional, or service-related limitations. Withdrawing consent may affect Northgate 55's ability to provide some services.
Where Northgate 55 is providing support directly to another capable adult, that person's consent will be respected regardless of whether a family member, caregiver, or other person believes that support would be beneficial.
Retention of Personal Information
Northgate 55 retains personal information only for as long as reasonably necessary to provide services, manage our business, meet applicable legal, regulatory, and professional requirements, and fulfill the purposes for which the information was collected.
Client records related to professional services are retained in accordance with the requirements of the Ontario College of Social Workers and Social Service Workers and applicable law. Generally, client records are retained for at least 10 years from the date of the last entry. Where the client was under 18 years of age at the date of the last entry, the record is generally retained for at least 10 years from the date the client became, or would have become, 18 years of age.
Other types of personal information may be retained for different periods depending on the nature of the information and applicable business, legal, or regulatory requirements.
When personal information is no longer required and applicable retention requirements have been satisfied, Northgate 55 will take reasonable steps to securely delete, destroy, or anonymize it, as appropriate.
A request to withdraw consent or discontinue services does not necessarily require the immediate deletion of existing records where Northgate 55 is required or permitted to retain those records.
Access & Correction of Personal Information
You may request access to personal information that Northgate 55 holds about you and may ask that inaccurate or incomplete information be corrected.
Northgate 55 will respond to requests in accordance with applicable privacy, legal, and professional requirements. In some circumstances, access may be limited or refused where permitted or required by law.
